USA Patriot Act Notice

To help the government fight the funding of terrorism and money laundering activities, federal law

(Section 326 of the USA Patriot Act) requires all financial institutions to obtain, verify, and record

information that identifies each individual that opens an account.

What this means to you: When you apply for a loan with Principle Mortgage Group, LLC (“360MG”), we will

ask you for your name, physical address, mailing address, date of birth, and other information that will

allow us to verify your identity. We may also ask to see your driver’s license and other indentifying

documents and copy or record information from each of them.

360MG will maintain the confidentiality of any information obtained in accordance with our privacy

policy and applicable laws and regulations.
